The Public Health Agency of Canada (PHAC) invites you to complete a short online survey about your experience with us – Family Matters Annapolis County family Resource Centre. This information will help PHAC improve programming across Canada.
Below are a few facts about the survey:
• It is being conducted by a company called Narrative Research on behalf of PHAC for all projects across Canada.
• It does not ask for any information that will directly identify you, such as your name or address.
• It takes about 10 to 15 minutes to complete.
• It should only be completed once. If you have already completed this survey, please do not complete it again.
• It will be available from January 7th to February 4th, 2025, to complete.
